Registration / Make an appointment

Central Scheduling – Making your appointment on the phone

If your doctor decides that you require a diagnostic test or a follow up appointment, you can call the Scheduling office and book your appointment date and time. Please wait 48 hours after your doctor’s appointment before calling the Scheduling office. This gives us enough time for your doctor to fax us the requisition and for us to process it. If your doctor has booked the date and time for your appointment and you need to change it, call the Scheduling office and have the appointment changed to a date and time that is more convenient for you.

At the time you call to schedule your appointment you can also complete a telephone pre-interview for registration. When you arrive at Registration on the day of your appointment, it will be much quicker once you have pre-registered by phone. You will be asked to validate your Ontario Provincial Health Card, and you will be asked if you have a cough, cold or fever before proceeding to the department to complete your scheduled test or appointment. No additional demographic or financial information will be required.

Registration – Once you arrive at the hospital

You must register for every hospital visit, and we are open 24 hours a day, 7 days a week. Depending on the date, time and reason for your visit you will be directed to the appropriate area when you arrive for your appointment.  We have 2 areas for registration at each hospital. In Owen Sound you will be directed to either Central Registration or our Emergency Department, and at the other sites, the Business Offices or the Emergency departments.

At Grey Bruce Health Services we are very aware that we need to capture all of your personal and health information correctly so that we can provide you with the best care possible.

When you come to register we will ask you for your Ontario Provincial Health Card along with your name, date of birth, address(es), phone numbers, emergency contact and next of kin and their contact information.  We will also ask for the name of your family doctor and/or Nurse Practitioner or Primary Care Provider. As well, we will need any Workplace Safety and Insurance Board (WSIB) information if your appointment is related to a work place incident. If you have additional health care insurance it would be helpful if you could supply us with your Group benefit and Policy number.
